For most people, an incurable illness would mark the end of ambition. For Cai Lei, it became the start of a far greater mission.
The 48-year-old former vice-president of Chinese e-commerce giant JD.com has been living with amyotrophic lateral sclerosis (ALS) for seven years. Today, the disease has reached an advanced stage, leaving him completely paralysed and unable to speak.
He communicates and continues his work only through eye-tracking technology, using his eyes to type and direct a team of researchers. Despite his condition, Cai works up to 12 hours a day, overseeing hundreds of research programmes aimed at finding treatments for ALS.
There are moments when the reality of his condition becomes life-threatening. In one incident, blocked airways left him unable to breathe for nearly a minute. Even then, he survived and continued his work without stepping back from his mission.
His determination is blunt and unwavering. &ldquo;Only death can stop me,&rdquo; he has said in interviews and public messages.
From Corporate Leader To Medical Research Pioneer

Before his diagnosis, Cai was one of China&rsquo;s most prominent tech executives, holding a senior role at JD.com. In 2019, his life changed completely when he was diagnosed with ALS.
Rather than withdrawing from public life, he redirected his energy, wealth and influence into medical research. He is now chairman of Beijing Aisikang Medical Technology and has reportedly invested more than 100 million yuan (around RM65 million) into ALS research.
Beyond personal funding, Cai has established multiple charitable foundations and collaborated with scientists and institutions to build research laboratories dedicated to understanding and treating the disease. His efforts have also helped create what is believed to be one of the world&rsquo;s largest ALS research databases.
That system now supports more than 300 drug development programmes, bringing together patient data, clinical research and experimental treatments in an attempt to accelerate progress in a field where breakthroughs have historically been slow.
Some early-stage therapies linked to these programmes have shown encouraging results in clinical settings, including improvements in muscle strength for certain trial participants.
Working Through Pain, Paralysis And Constant Care

Cai&rsquo;s daily reality is defined by extreme physical limitation. He is fully dependent on carers for movement, with four required to assist him in basic tasks such as being lifted from a chair or moved to bed.
Muscle atrophy has made prolonged sitting painful, while even minor complications can become dangerous. He has reportedly survived multiple near-death episodes, including a serious breathing incident caused by airway blockage.
Despite this, he continues to manage research operations through eye-tracking systems and digital communication tools. Every instruction, every decision, is made through controlled eye movements translated into text.
His work is not limited to drug development. Cai has also helped establish platforms that connect ALS patients directly with caregivers, aiming to reduce costs and prevent exploitation in the healthcare system. The initiative was designed to make care more transparent and accessible, particularly for families struggling with both medical and financial pressure.
A Larger Impact On ALS Research

Medical professionals in China have described Cai as an unusual figure in the field of neurology, not only because of his condition but because of his direct involvement in advancing research while being a patient himself.
Some experts have credited him with helping open new research pathways in ALS studies. His team&rsquo;s data-driven approach, combined with artificial intelligence tools, has reportedly sped up certain drug discovery processes.
More than 30 projects under his broader network have entered clinical trials, while dozens more remain in earlier stages of development. Although ALS remains incurable, these efforts represent incremental progress in a field where advancement is typically slow and difficult.
What Exactly Is ALS?
Amyotrophic lateral sclerosis (ALS), also known as motor neurone disease, is a progressive neurological condition that affects nerve cells in the brain and spinal cord.
These nerve cells control voluntary muscles, including those used for movement, speech, swallowing and breathing. As ALS progresses, the cells gradually deteriorate and die, causing muscles to weaken and waste away over time.
Patients slowly lose the ability to walk, speak and eat, and eventually require assistance to breathe. One of the most devastating aspects of ALS is that many patients remain mentally aware throughout the progression of the disease, even as their bodies become increasingly immobile.

Many people around the world were first introduced to ALS through the viral Ice Bucket Challenge in 2014, which saw millions of participants &mdash; including celebrities, athletes and world leaders &mdash; pour buckets of ice water over themselves to raise awareness and funds for ALS research. The campaign generated hundreds of millions of dollars for research efforts and helped accelerate scientific studies into the disease.
Despite the increased awareness and advancements in research over the past decade, there is still no cure for ALS. Existing treatments can only help slow the disease's progression or manage symptoms, making ongoing research efforts, such as those led by Cai Lei and other scientists around the world, crucial in the search for more effective treatments and eventually a cure.
A Mission Larger Than Himself 
Behind Cai&rsquo;s journey is the support of his wife, Duan Rui, a pharmacy graduate. She manages a social media account called Breaking the Ice Station, where she raises funds for scientific research through livestreams and product sales.
She has spoken about the emotional weight of watching her husband&rsquo;s condition deteriorate while continuing to fight for others.
&ldquo;I still fear parting, but because of that, I cherish every second I spend with him,&rdquo; she said.
Cai&rsquo;s story has resonated widely across social media in China, where many have praised his resilience and refusal to give up. For supporters, his mission has become more than a personal struggle&mdash;it represents a broader hope that scientific progress can eventually defeat a disease long considered untouchable.
Even as his body continues to fail, Cai remains focused on one goal: to help ensure that future ALS patients may one day have a different ending to their story.
